Abstract

Placental abruption is the premature separation of the placenta from uterus. Pathologically, it occurs while the fetus is in uterus, which can result in fetal mortality, prematurity and maternal morbidity. Couvelaire uterus is an extravasation of blood that infiltrates the wall of the uterus. As a result, the myometrial muscles can not contract and cause postpartum hemorrhage. This is a case report of a patient at 32 weeks gestation with  gestational hypertension without treatment and smoker who presented to the hospital with abdominal pain and frontal headache, without fluid loss or vaginal bleeding. The patient was diagnosed with fetal death and placental abruption after obstetric ultrasound. After fetal expulsion, the patient developed uterine atony and puerperal hemorrhage, requiring hysterectomy. Couvelaire uterus was diagnosed by direct visualization.
