The use of Transcutaneous Electrical Diaphragmatic Stimulation in Postoperative Myocardial Revascularization

Abstract

Nowadays, the cardiac surgery for myocardial revascularization has been frequently executed as a consequence of the great number of coronary disease cases. It is characterized as a major surgery which can be followed by a few complications in its postoperative, including the diaphragmatic paresis. Physiotherapy can be beneficial to this effect and the transcutaneous electrical diaphragmatic stimulation (TEDS) is one of the available physiotherapeutic resources. Therefore, the aim of this study was to analyze and compare the variation of inspiratory muscle strength and pulmonary function with spirometric parameters such as CVF, VEF1 and IT during the following periods: preoperative, 2nd postoperative and 5th postoperative days. Five individuals who undertook myocardial revascularization were analyzed and treated. A physiotherapeutic protocol in association to transcutaneous electrical diaphragmatic stimulation (TEDS) was carried out. Besides the pre-elaborated index cards, the evaluation consisted of spirometry and pressure manometry, which were preformed again on the 2nd and 5th postoperative. The treatment was started on the 2nd postoperative day. As a result, it was observed on the 2nd postoperative a reduction of all the evaluated parameters and there was an increase on the 5th postoperative while comparing to the 2nd postoperative. Therefore, we can conclude that TEDS can be recommended to the treatment of these individuals in association to the physiotherapeutic treatment. However, due to the sampling, it might be that the results were modified, being highly important that other studies and evaluation methods be carried out.
