Functional laryngectomy, quality of life, and speech-language pathology challenges: a clinical case study

Abstract

 

Implications for the patient’s quality of life have become crucial in recent treatment decisions. This single case study compared the pre- and post-total laryngectomy periods, focusing on quality of life and functional aspects. Assessment instruments such as the Vocal Handicap Index, M.D. Anderson Quality of Life Questionnaire, University of Washington, and World Health Organization Disability Assessment Schedule were used, as well as speech samples and videofluoroscopy swallowing exams The findings indicate that laryngotracheal separation was beneficial following head and neck cancer treatment, facilitating oral intake and aiming to restore the pleasure of eating and alaryngeal communication. It can be concluded that the improvement in quality of life was evident, and the permanent stoma did not negatively impact the patient’s life.

Graduated in Speech Therapy from the Pontifical Catholic University of São Paulo (2022). Participated in the Education through Work Program for Health - PET Health Management and Assistance (2022). Worked as a Speech Therapist at ABADS - Brazilian Association for Assistance and Social Development (2023); Worked as a Preceptor for the Pontifical Catholic University of São Paulo in the public health service, namely, the Child and Youth Psychosocial Care Center (CAPSIJ-II) and the Basic Health Unit (Ubs) (2023). Currently a master's student in Human Communication and Health at PUC-SP, and responsible speech therapist at - AP. FONOAUDIOLOGIA LTDA.

Lívia Fernandes Barata de Carvalho, AC Camargo Câncer Center

She is currently a hospital speech therapist at AC Camargo Cancer Center, with a master's degree (2010) and doctorate (2017) in Oncology Sciences from AC Camargo Cancer Center - Fundação Antônio Prudente - FAP; CAPES scholarship holder. Specialist in Orofacial Motricity with a focus on Dysphagia from AC Camargo Cancer Center and in Voice from the Center for Voice Studies - CEV. Reviewer for the Journal of Communication Disorders.

Renata Ligia Vieira Guedes, University Center of the United Metropolitan Colleges

Graduated in Speech Therapy from the State University of Health Sciences of Alagoas - UNCISAL (2005). Specialist in Orofacial Motricity in Oncology (2007) and in Dysphagia (2007) from the Antônio Prudente Foundation, AC Camargo Hospital - São Paulo/SP. Master and PhD in Sciences - concentration area Oncology, from the Antônio Prudente Foundation, A.C. Camargo Hospital - São Paulo/SP (2008/2010). Post-doctorate from the University of Florida (UF), under the guidance of Dr. Ianessa Humbert at the Swallowing Systems Core Laboratory (SSC) (2016/2017). Member of the Brazilian Society of Speech Therapy, Brazilian Academy of Dysphagia, Dysphagia Research Society and European Society of Swallowing Disorders. Marcus Vinícius Furlan, The Portuguese Charity of São Paulo

Graduated in Medicine from the University of Ribeirão Preto (2009). Medical Residency in General Surgery from Casa de Saúde Santa Marcelina de Itaquera (2012) and Head and Neck Surgery from A.C.Camargo Cancer Center (2014). Specialist Title from SBCCP/AMB (2014), Effective Member of the Brazilian Society of Head and Neck Surgery, Corresponding Member of the American Thyroid Association - ATA and Ordinary Member of the European Laryngological Society - ELS. Dedicated to oncological surgery of the Head and Neck and minimally invasive surgeries of the thyroid, oropharynx and larynx.

Leslie Piccolotto Ferreira, Pontifical Catholic University of Sao Paulo

She holds a degree in Speech Therapy from the Pontifical Catholic University of São Paulo (1971), a master's degree in Linguistics Applied to Language Teaching from the Pontifical Catholic University of São Paulo (1981), and a PhD in Human Communication Disorders (Speech Therapy) from the Federal University of São Paulo/Paulista School of Medicine (1990). She is currently a full professor in the Department of Theories and Methods of Speech Therapy and Physiotherapy at the Pontifical Catholic University of São Paulo. In the Voice, Communication, Health and Art Research Line, she teaches classes and supervises research on professional voice, mainly related to teachers' voices. This work contributed to discussions for the recognition of Work-Related Voice Disorder (WRVD) and the consequent publication of the WRVD Protocol by the Ministry of Health. She was president of the Regional Council of Speech Therapy (1989-92) and of the Brazilian Society of Speech Therapy (2002-3). She was Vice-President of the Brazilian Society of Speech-Language Pathology and Audiology - 2017-2019. She is currently the Executive Editor of the Journal of Communication Disorders and Coordinator of the Voice Laboratory (LaborVox) at the Pontifical Catholic University of São Paulo. She works as a reviewer or member of the Editorial Board of the journals CEFAC, CoDAS, Audiology Communication Research, and Saúde em Revista.
